For how many days / weeks / months, maternity leave is permissible?

Generally, maternity leave is permissible for 90 days or if you count in weeks then its 12 weeks. Similarly, in months, maternity leave is permissible for 3 months. But, in some special cases e.g pregnancy problem or any other health issue, it may be extended.

With due respect it is stated that I am Hina Akbar working as SESE Science under your supervision, in School Education Department at GHS Mahni Loka. It is to inform you that I want to avail the facility of maternity leave, given to female staff as per the policy of our department. Kindly, make it possible and grant me leave for three months dated as 1st June to 1st August. It would be a great favor for me.

To Whom Submit Application?

Amendment in this regard issued on 31-10-2012, clearly says that the sanction authority for maternity leave is immediate in-charge i.e Headmaster or Principal etc. (Check here on this link)

What Documentation is Required with Application?

Generally, with maternity leave application, you have to attach only your pregnancy positive report. Although, your in-charge or appointing authority may require additional documentation too with your application. Therefore, just submit your application with pregnancy positive report and be relaxed.
